FREE EYE EXAM AND EYE GLASSES



Last year we referred 82 students to this program and 64 students received FREE glasses.

My goal this year is to refer 100 students but I need your HELP…

Do you know a student who needs a FREE EYE EXAM AND EYE GLASSES???

Sweetwater High has been selected to participate in a free eye clinic that will provide students with free eye exams and eyeglasses.

The National City Eye Clinic will be held November 10-14, 2014 at National City YMCA at Las Palmas Park located at 1810 East 22nd Street, National City, CA 91950. 

If you know a student who could benefit from this program:
· You can print the forms for them (see attached) OR
· They can pick up the forms in the health office
· They can download the forms here:

Only students who are REGISTERED and have a signed consent will be able to participate in the program.

The LAST day to register is October 27th.
